We’re excited to have you on board with us.

The following lessons will help you understand our program — our internal policies, the requirements of the program as laid out by our Lead Health Homes, the State and federal governments, as well as best practices for supporting our members and working with all of our different community partners.

The lessons are laid out to have portions of our Housing Works Health Home Policy and Procedure Manual, followed by a corresponding lesson that is either required by one of our Lead Health Homes or is material that reinforces understanding our policies and procedures.
