Welcome to Health Home at Housing Works!

We’re excited to have you on board with us.

The following lessons will help you understand our program — our internal policies, the requirements of the program as laid out by our Lead Health Homes, the State and federal governments, as well as best practices for supporting our members and working with all of our different community partners.

The lessons are laid out to have portions of our Housing Works Health Home Policy and Procedure Manual, followed by a corresponding lesson that is either required by one of our Lead Health Homes and reinforces many of our policies and procedures.

We know you’ll do your best to take in all the information in these trainings. You can always return to this course and lessons as resources and if questions arise, make sure to take note of them to ask your supervisor.
